Adding Wellbutrin to an SSRI for Depression

The "Residual Symptom" Problem

Many patients on a standard SSRI (like Lexapro, Zoloft, or Prozac) achieve good results for depression, but may not fully get to where they want to be. They are left with what we call residual symptoms:

They may also experience side effects related to the SSRI, such as sexual side effects (low libido, delayed orgasm, erectile dysfunction) and weight gain.

Enter Wellbutrin (Bupropion): The Dopamine-Norepinephrine Boost

While SSRIs work primarily on serotonin, Wellbutrin targets dopamine and norepinephrine. When added to an SSRI, it often acts as a "booster" to fill in the gaps that serotonin alone cannot reach.

Why We Use This Combination:

  1. Combating Fatigue: Wellbutrin is "activating." It can help clear the morning grogginess or lethargy often associated with SSRIs.

  2. Addressing Sexual Dysfunction: Wellbutrin is one of the few antidepressants that does not cause sexual side effects; in fact, it is often used as an "antidote" to SSRI-induced libido issues.

  3. Weight Neutrality: Unlike many SSRIs which can lead to modest weight gain, Wellbutrin is weight-neutral or may even lead to slight weight loss.

  4. Cognitive Precision: By boosting norepinephrine, it can help with the "executive dysfunction" and lack of focus that depression often leaves behind. This is why it is also a non-stimulant treatment for ADHD.

Is it Safe? (The Clinical Nuance)

As a Board-Certified Psychiatrist, I carefully screen for a few specific factors before using this "dual-action" approach:

  • Anxiety Threshold: Because Wellbutrin is stimulating, it can occasionally increase jitteriness in patients, especially those with primary Panic Disorder or another anxiety disorder.

  • Seizure Threshold: Higher doses of Wellbutrin are contraindicated in patients with a history of seizures or eating disorders (Bulimia/Anorexia).

  • Sleep Patterns: Timing is key. Wellbutrin is almost always taken in the morning to prevent insomnia.

Tailoring Your Treatment Plan

There is no "one size fits all" in psychopharmacology. The goal of adding Wellbutrin isn't just to add another pill, it’s to achieve a precise clinical synergy that allows you to feel like yourself again. Sometimes a combination approach is more effective than continually changing out one medication for another.
